Fieldgate Dentistry provides general and specialized dental care services. All treatments are performed by licensed dental professionals in accordance with the standards of the Royal College of Dental Surgeons of Ontario.

Appointments

• Appointments can be booked by phone or through our online booking system.
• Please provide at least 24 hours notice for appointment cancellations.
• Repeated no-shows or late cancellations may result in a cancellation fee.
• We strive to see all patients on time but emergencies may occasionally cause delays.

Payment Terms

Payment is due at the time of service unless other arrangements have been made.

• We accept cash, credit cards, debit cards, and insurance direct billing where applicable.
• Patients are responsible for any amounts not covered by insurance.
• Payment plans may be available for extensive treatments.

Insurance

We work with most major dental insurance providers. However, patients are responsible for understanding their coverage and any co-pays or deductibles. We will submit claims on your behalf but cannot guarantee coverage amounts.

Patient Responsibilities

• Provide accurate and complete medical and dental history.
• Inform us of any changes to your health or medications.
• Follow post-treatment instructions and care recommendations.
• Attend scheduled follow-up appointments.
• Maintain good oral hygiene practices at home.
